Additional Community Resouruces Representatives from the Southern Nevada Health District and the Secretary of State’s office will also be on site April 24 to offer their services including:
- Rapid HIV testing, safe sex kits,
- harm reduction supplies
- Voter registration information
- Business registration
- Notary services

To apply for your U.S. passport, you must:
- Submit your completed application, DS-11, to request the Passport Book, Passport Card, or both; or submit the Renewal form, DS-82, to renew your passport. The application forms DS-11 and DS-82 can be downloaded from this website: travel.state.gov.
- Submit your original evidence of U.S. Citizenship:
- A certified copy of your birth certificate;
- An original Certificate of Naturalization or Citizenship;
- An original Report of Birth Abroad; or
- A previously issued, undamaged U.S. passport.
- Bring a photocopy of your evidence of U.S. Citizenship.
- Submit one recent Passport Photo – 2 inches x 2 inches. No glasses in photos.
- Present valid Photo Identification – Driver’s License, State ID Card, current passport, or Military ID, and submit a photocopy of the front and back of that ID.
- Payment of passport fee by credit/debit card, check, and/or money order.
